Fountas & Pinnell Assessment Adapted Recording Forms Teaching Students to Print
   

 

The DLC has adapted the layout of the Fountas & Pinnell Assessment Recording Forms in order to reduce the amount of paper required.   
One page from each Running Record form can be eliminated by adding the Summary Label and Accuracy Rate Continuum Label:
• Assessment Summary labels may be placed in the top right-hand corner space.
• The Accuracy Rate labels may be placed at the bottom of the first page.

These adaptations allow you to have the information on the front page, rather than at the back. Download these adaptations onto label paper:
Summary Labels –use 2 5/8” x 1” size labels;  (30 per sheet)
Accuracy Rate Labels –Avery 06503 (full sheet 8 ˝” x 11”).   Use a copied Running Record Recording Form as your master.
The Benchmark Assessment System Reference Card provides criteria for accuracy levels, a comprehension rubric and a fluency scoring key.  Download the Reference Card and laminate for durability.
