A CAP Summary sheet places all the bar charts (the 5 math scores, the 6 areas of ELA) for a student on a single page. It should only be requested for late arriving students - you will receive the Student Profile sheets later in the school year which will contain this year's and last year's CAP data as well as CGC data, etc.
If you would like to receive the CAP 2003/04 summary sheets for students who have moved into your school from another Division school print out this page and return it in the courier to:
